Below is the Central Station Call List form.
Please print clearly to avoid errors and ensure the following information is included:
- Name, Acct. #, phone, address and email
- Please indicate order you want contacts called, for example 1. my cell 2. my wife’s cell 3. police 4 next contact on list. The reason we need the proper order is that the central station needs to know exactly what order they need to call the police.
- Indicate whether the contact phone numbers are cell, home or business.
- Code word is required.
- This form may be emailed or faxed but the original signed copy must be mailed to Banner Alarm as per Central Station requirements.
Call lists received after 12 pm will be processed the next business day.
