Below is the Central Station Call List form.

Please print clearly to avoid errors and ensure the following information is included:

• Name, Acct. #, phone, address and email
• Please indicate order you want contacts called, for example 1. my cell 2. my wife’s cell 3. police 4 next contact on list. The reason we need the proper order is that the central station needs to know exactly what order they need to call the police.
• Indicate whether the contact phone numbers are cell, home or business.
• Code word is required.
• This form may be emailed or faxed but the original signed copy must be mailed to Banner Alarm as per Central Station requirements.
